Providence ElderPlace is a federally recognized Program of All-Inclusive Care of the Elderly (PACE) which offers a seamless provision of total care. The BSW Social Worker functions as a member of the interdisciplinary team and represents social work perspective regarding geriatric participant issues to the interdisciplinary team. As a member of the interdisciplinary team, this position provides social services to PACE Participants, family, caregivers, and decision-makers in a manner which is consistent with the mission and core values of Providence Health System. Provides casework and resource assistance to enhance the overall psychosocial functioning of persons being served. Collaborates closely with the Interdisciplinary Team and the patient, family, and other health care professionals to develop and implement effective interventions, supports, and resources to ensure continuity of care across the continuum. Conducts social work visits and interventions in other care settings, including the participant's home, as needed.
