Brief description of placement:
This placement will support patients receiving treatment in UWMC-ML Outpatient Oncology Clinics, primarily in response to patient distress screenings results as part of a team that includes social work, registered dietician services and palliative care.
Learning opportunities and responsibilities will include psychosocial assessment and resource coordination for oncology patients and their families. Areas of need can include but are not limited to housing, finances, transportation barriers, mental health, diagnosis adjustment, caregiver support, grief and loss, and interpersonal violence/safety concerns. Provides care coordination with outside agencies as appropriate, including CPS/APS, Offers consultation and guidance to the oncology clinic providers for complex social needs.
Professional written communication skills and timely documentation required.
