Description

The acclaimed Swedish Neuroscience Institute (SNI) provides advanced, progressive treatment for a wide range of brain, spine and central nervous system conditions. We have a roster of world-class neurologists and neurosurgeons, leading-edge operating rooms, Telehealth access for patients outside the area, and a specially-trained Inpatient Neurology Team – all with a level of care and compassion focused on improving outcomes and renewing hope.

Micro

• Trainees placed in a SNI clinics will gain experience working in an integrated outpatient neurology setting.

• Helping the SNI social workers with various clinical tasks, such as:
-Reviews clinic chart and identifies patients and their families in need of social work services. Conducts pre-assessment and triages referrals among SNI social work team, as needed when managing SNI Referrals.
-Completes psychosocial assessment to determine patient and family strengths and needs relative to living and coping optimally with diagnosis, treatment, follow-up care. Includes gathering data about: patient and family
psychosocial strengths and coping skills; support systems / care giving needs; ethnic, spiritual and cultural influences; functional status; housing, financial, insurance, legal and transportation systems.
-Provides interventions designed to address current and future problems as the patient’s medical and
psychosocial needs evolve. Includes advocacy, coordination of care giving, housing, financial, insurance, legal and transportation resources; community and other resources and programs; and case management services.
- Develops a case plan with patient and family based upon mutually agreed upon goals to enhance, maintain and promote optimal psychosocial functioning throughout the patient’s continuum of care. Participates in multidisciplinary care planning with patient’s health care team and the patient and their family. Communicates psychosocial progress to physicians and other multidisciplinary team members.

Mezzo

• Coordinating care between clinics within the Swedish system as well as coordinating with outside resources.
• Other opportunities of interest as they arise (e.g. Attendance at Swedish sponsored trainings/CME’s, research participation, etc.).

Required Experience

Completion of foundation year MSW program. Healthcare experience preferred, but not required.
