Providence ElderPlace is a federally recognized Program of All-Inclusive Care of the Elderly (PACE) which offers a seamless provision of total care. Providence ElderPlace is both a health insurance and a health care provider. Strives to keep older adults as healthy as possible living in the community through clinics, home visits and more.
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

- provides direct service activities within the center
- 1 on 1 and group psychosocial counseling
- addressing social issues that impact marginalized patients to thrive independently
- conducts comprehensive assessments targeting social, emotional, cognitive and ethnic factors to make individualized integrated plan of care
- assess community and financial resources to support continuity for at-risk patients
- outpatient (home visits)
- collaborate with interdisciplinary medical team and make clinical observation and decisions based on social work practices and ethics
- coordinate discharge planning for hospitalizations
- understand and facilitate housing placements for patients with higher level of care needs
- assist with hospice and end-of life transition for patients with terminal illnesses
- support patients family by education of respite care and resources
