We work to remove barriers to health for all people, so that no one faces illness and injustice alone. Our comprehensive services include food and nutrition, healthcare navigation, aging and disability support, and connection to housing and other vital resources.
A student will get an opportunity to meet and work with talented teams who make real, meaningful changes in the daily lives of those living with chronic and life-threatening illnesses in their community.
Micro
In the fall, a student will reach out to clients to help them with their health insurance enrollments and shadow Medical Case Managers with intakes, Individual Service Plan, Comprehensives Assessment, and health insurance and other benefit enrollments. In Winter, student will oversee a client case load of between 6-12, build rapport with clients and support them in identifying, prioritizing, and meeting their needs. A student will create Individual Services Plans as well as completing comprehensive Assessments (Psychosocial Assessments) with clients. Other examples of common case management activities include benefit renewals, such as the Early Intervention Program, which helps clients pay copays. In the spring Students design, conduct and present research about a topic of their choice.
Mezzo
Students participate in weekly team meetings, support group meetings, Multi-Disciplinary Team (MDTs) meetings, monthly case consultations, Outside supervision and skills development trainings, and other related group activities.
Macro
Students attend appropriate planning and steering committee meetings available which affect statewide programs, EIP (early Intervention Program) meetings, King Count Metro Meetings, Research, and other related activities.
