Micro
As a GUIDE Care Navigator, you will play a vital role in supporting individuals living with dementia, including Alzheimer’s disease, and their family caregivers. You’ll be part of an innovative new program, launched through the Centers for Medicare & Medicaid Services (CMS), designed to provide comprehensive, wraparound services to families navigating this journey.
Your primary responsibilities will include:
- Visiting families in their homes to conduct biopsychosocial and safety assessments
- Building strong rapport with clients and caregivers
You will collaborate within our interdisciplinary team, which includes physicians, nurse practitioners, neuropsychologists, social workers, and fellow care navigators, ensuring families receive holistic, high-quality care.

Mezzo
A core pillar of Magnolia’s care model is collaboration with community-based organizations, ensuring that families affected by Alzheimer’s disease and related dementias receive the support they need beyond the walls of a clinic. By bridging the gap between healthcare and community resources, Magnolia helps create a more comprehensive and accessible system of care.
As part of this practicum, you will play an active role in shaping these partnerships, working to streamline and operationalize key aspects of the GUIDE program. Your insights and recommendations will help refine the program, enhancing its impact and accessibility for families. You'll also have the opportunity to attend collaborative networking meetings, connecting with key stakeholders and contributing to the growth of a stronger, more integrated support system.
Through this work, you will be part of a movement that reimagines how healthcare and community services come together, ensuring that families navigating dementia receive the wraparound care they truly need.
Macro
As a brand-new pilot program, it will be essential to take an advocacy-driven approach, identifying gaps in care as we roll out this initiative across Washington. A key part of this role will be understanding the policies that impact these services and recognizing where improvements are needed.
This practicum also offers an opportunity to participate in Advocacy Day, where you can actively engage in shaping state-level initiatives that support older adults and their care partners. By amplifying the needs of this community, we can help ensure that Washington continues to prioritize essential resources and policies that enhance care and accessibility.
Required Experience
Preferred to have experience (professionally or lived) working with older adults and aware of issues affecting family caregivers.
