Description

UW Medicine Primary Care operates primary care clinics located throughout the Puget Sound region. These clinics offers a wide spectrum of primary care services including a behavioral integration program (BHIP).

BHIP is an evidence-based integrated care program that offers behavioral health care for persons in the primary care setting.  This treatment model offers a "team" approach that includes collaboration with primary care doctors, consulting psychiatrists and clinical social worker. The Social Worker in this position is responsible for supporting the overall behavioral health care of the patient in consultation with the team psychiatrist and the patient’s primary care provider. The BHIP Social Worker is a hybrid role comprised of clinical social work, care coordination and general social work.

Micro

Provide outreach and engagement to patients. Provide BHIP orientations to patients. Facilitate use of clinical screening tools with patients. Complete clinical assessment, treatment planning and provision of clinical interventions to patients. Assist with crisis intervention. Assist with triage and clinical intervention formulation for patients.

Mezzo

Assist with monitoring/analysis of clinical metrics obtained from patients of UW Medicine Primary Care BHIP Program. Develop knowledge and understanding of relevant evidence based clinical interventions. Develop knowledge and understanding of systemic barriers impacting patients.

Macro

Develop in depth knowledge and understanding of equity, diversity, and inclusion factors with specific emphasis on healthcare equity. Develop in depth knowledge and understanding of relevant ethical guidelines and professional conduct pertaining to role within UW Medicine Primary Care including NASW Code of Ethics, WACs and UW Medicine policies and trainings.

Required Experience

Previous healthcare experience preferred.
