Description

Seattle Children’s South Clinic provides outpatient specialized care to infants, children, teens, and young adults across the South Sound region and including Montana, Alaska, and Eastern Washington. South Clinic serves over 20 specialty areas including Neurodevelopmental, Cardiology, Endocrine, Nephrology, Adolescent Medicine, Sports Medicine, etc. This student placement is under the direct supervision of two clinical social workers who provide medical social work and case management support to individuals, families, and groups. One of whom specializes in Adolescent Medicine.

This placement will provide the student with a unique range of opportunities that could help inform the direction/ interests in their future medical SW journey.

COMMON DAILY TASKS

Attending daily huddle, meeting with patients/families in clinic to address topics such as DDA, SSI, School Accommodations, SDOH resources, adjusting to new diagnoses, and family/ caregiver support. Collaborating with multidisciplinary teams in clinic to assess and address needs. Daily documentation; use of electronic health records, and review of internal security alerts for patients seen in clinic, day of.

SKILLS/ INTERVENTIONS

Biopsychosocial assessments, safety planning, resource coordination, advocacy, outreach out community programs such as DDA, writing chart notes in EMR, brief mental health interventions and screenings, suicidal ideation screenings, and problem solving.

Micro

Meeting with individual patients and their caregivers; psychosocial assessments; resource coordination; basic needs; safety planning.

Mezzo

As a social work student you will serve as a liaison between Seattle Childrens Hospital and various community entities, including DCYF, Schools, legal systems, and medical organizations outside of SCH. Coordination of psycho-social response with all members of the care team (including providers, physical therapists, MA’s, RN’s, and registration). Refer families to community resources that address social determinants of health (SDOH), such as food banks, homeless shelters, financial assistance, etc.

Macro

Engaging with Diversity, Equity, Inclusion initiative and efforts in healthcare setting. As a student you will have the opportunity to assist families with Federal and state program resource connection and navigation. Plenty of opportunities for advocacy.
